* Right To Object To or Request For Restrictions. You have the right to object to or ask for a restriction or limitation on the medical information we use or share for treatment, payment or health care operations. You also have the right to ask for a limit on the medical information we share with someone who is involved in your care or in the payment for your care. Such a person may be a family member or friend. We do not have to agree to your request when: (i) disclosure is necessary to protect your life and health and you are no longer able to express consent to the disclosure, (ii) disclosure is necessary for the purpose of medical treatment, and (iii) disclosure is necessary for the protection of lawful rights and interests of natural or legal persons in court proceedings, or the establishment, exercise, or defense of legal claims, or when provided to government or public authority pursuant to a constitutional or statutory mandate. If we do agree, we will fulfill your request. To ask for restrictions, you must make your request in writing on a form that we will give you upon request. You must tell us:  

  • what information you want to limit, 

  • how you want us to limit the information, and  

  • to whom you want the limits to apply.

Hindi naminkailangang sumang-ayon sa iyong kahilingan kung: (i) ang pagbahagi ng iyong impormasyon ay kinakailangan upang maprotektahan ang iyong buhay at kalusugan at ikaw ay wala nang kakahayan maipahayag ang paghintulot, (ii) ang pagbahagi ay kinakailangan upang matugunan ang pagbigay ng kinaukulang medikal na paggamot, at (iii) angpagbahagi ay kinakailangan upang maprotektahan ang karapatan at interes ng mga taong may kaugnayan  sa paglilitissa korte, o ang pagtatatag,pagpapatupad, o pagtatanggol sa mga legalna paghahabol, o kung ibinigay sa pamahalaan o pampublikong awtoridad alinsunodsa isangkonstitusyon o batas.Kung kamiay sumasangayon, tutuparinnamin angiyong kahilingan.Upang humilingng mgapaghihigpit, kailangang ipagbigay-alam sa amin ito sa pamamagitan ng pagsagot sa aming form naibibigay sa iyo.   • anong impormasyonang gustomong limitahan, 

  • paano mo nais nalimitahan namin angimpormasyon,

    * Right To Block/Remove. If warranted such as when the information is false, unlawfully obtained, or being used for purposes you did not authorize, you have the right to suspend, withdraw, or order the blocking, removal, or destruction of your personal information.  

     

    * Karapatan Hindi Makita o Tanggalin. * Right To Inspect and To Receive Copies. You have the right to look at and to receive copies of the medical
information used to make decisions about your care. Usually, this includes medical and billing records. It does not include some records such as psychotherapy notes. To look at and to receive copies of medical information used to make decisions about you, you must submit your request in writing. We may charge a fee for the costs of processing your request. Contact CARESPAN customer service for more information. 

 

* Karapatang Mag-inspeksyon at Tumanggap ng mga Kopya. May karapatan kang tumingin at tumanggap ng mga

kopya ng medikal na impormasyon na ginamit upang gumawa ng mga desisyon tungkol sa iyong pangangalaga. Karaniwan, kabilang dito ang mga talaan ng medikal at pagsingil. * Right To Amend or Correct. If you think that medical information we have about you is wrong or incomplete, you have the right to ask for an amendment to your record. To ask for a change to your record, you must make your request in writing and submit it to CARESPAN customer service. Also, you must give a reason that supports your request.  

 

* Karapatan Upang Susugan o Itama. Kung sa tingin mo ay mali o hindi kumpleto ang medikal na impormasyong mayroon kami tungkol sa iyo, may karapatan kang humingi ng amendment sa iyong record. * We may say no to your request for an amendment to your record, if such request is vexatious or unreasonable. We may do this if it is not in writing or does not include a reason to support the request. We also may say no to your request if you ask us to amend information that:  

 

    • we did not create, unless the person or entity that created the information is no longer available to make the amendment;  

    • is not part of the records used to make decisions about you;  

    • is not part of the information which you are permitted to inspect and to receive a copy; or is accurate and complete.

CARESPAN is a company that provides an Internet-based healthcare clinic that lets the consumer consult with their physician via real time live video feed, keeps an electronic health record for patients and their doctors, and supplies various hardware and software tools to patients and doctors to facilitate medical care. In the course of our business, we would need certain personal information so that we will be able to deliver our commitments in a timely, effective, and accurate manner. 


What Information This Policy Covers 

By agreeing to provide us such information, you consent and agree that we may collect, use, and disclose such data in the pursuit of delivering our services and to all other purposes that are permitted or required by law. If you choose not to provide us with any required Information, we may not be able to provide you with any of the services that our organization offers. 


What Information We Collect 

The following information will be collected to facilitate your account creation: 

  • Full Name 

  • Residential Address 

  • Date of Birth 

  • Telephone and/or Mobile Numbers 

  • Emergency Contact Information 

 

To accurately and efficiently provide medical diagnosis by the Medical Professionals and practitioners, the following may also be collected: 

  • Full Medical History 

  • Prescribed In-use or previously used medication 

  • Medical examination or tests results 

  • Other pertinent documents required by Medical Professionals and practitioners. 


How We Use the Information 

As mentioned, we will use this information in the course of providing the services that we offer, furthermore and more specifically for the following purposes: 

  • Identification of Patients 

  • Billing Purposes 

  • Medical Diagnosis 

  • Other requirements of the Medical Professional  

  • Communicating with us, via email message or otherwise to contact us, we may use the information provided by you to respond to your communication and/or as described in this Privacy Policy, we may also archive this information and/or use it for future communications with you. 

  • Optionally and only upon your approval and consent, we may use these Information collected when you sign up for our various email or update services to send you the messages in connection with our offerings and specific areas of our website or other social media platforms. 


Who Uses the Information 

The aforementioned information will be used by various personnel in our organization who are directly involved in that particular engagement as well as other office backend related functions and may include one or all of the following: 

  • Medical Professionals and Practitioners 

  • Finance, Logistics, and Legal 

 

Furthermore, all of our Employees, Sub contractors, and external parties that may need access to such information are mandated to sign a comprehensive Non-Disclosure Agreement/contract with us separately. 


Sharing the Information 

The Information that we obtain, except as set forth in this Policy or as specifically agreed to by you, will not be disclosed nor sold or exchanged to any third party. In addition to the other uses set forth in this Policy, we may disclose and otherwise use them to entities that we have a comprehensive Non-Disclosure Agreement or Contract to and as described below: 

  • Communications with Our Business Partners. We may share your Information with business partners sights to receive communications from such parties that you have opted into. 

  • Disclosure to Operations, Contractors, and Partners. Our operations, contractors, and partners may have limited access to your Information in the course of providing products or services to or through us such as other medical practitioners and professionals, laboratories, hospitals, and other health provider information. The vendors and suppliers that provide us with technology, services, and/or content related to the operation and maintenance of our system or the Products and Services that we carry. Access to these Information by these contractors is limited to the information reasonably necessary for the contractor to perform its limited function for us. 

  • Government Authorities, Legal Rights and Actions. We may share your Information with various government authorities in response to subpoenas, court orders, or other legal process to establish or exercise our legal rights or to protect our property to defend against legal claims or as otherwise required by law. In such cases, we reserve the right to raise or waive any legal objection or right available to us. We also may share your Information when we believe it is appropriate to investigate, prevent, or take action regarding illegal or suspected illegal activities to protect and defend the rights, property, or safety of our Organization, the App, our users, customers, or others and in connection with our Terms of Use and other agreements. 

  • Disclosure to Acquirers. CareSpan Lite may disclose and/or transfer your Information to an acquirer, assignee or other successor entity in connection with a sale, merger, or reorganization of all or substantially all of the equity, business or assets of CareSpan Lite to which your Information relates. 

 

Confidentiality and Security of Your Information 

We consider the confidentiality and security of your information to be of the utmost importance. We will use commercially reasonable efforts to keep your information private and will not share it with third parties, except as otherwise provided in this Privacy Policy, or unless we have good faith belief that such disclosure is necessary in special cases, such as a physical threat to you or others.  

 

CareSpan Lite takes reasonable measures to protect your Information from disclosure to or access by third parties. We host our facilities in Data Centers that are compliant with Globally recognized Standards and best practices. We employ encryption of your data in transit and at rest in addition to our subscription of 24x7 Managed Network and Security Operations Monitoring Systems. Please do note that no one can 100% guarantee the security of Information as there will always be the risk and possibility that an unauthorized third party may find a way to circumvent our security systems or that transmission of your information over any communication channel may be intercepted. 


Updating or Deleting Your Information 

We maintain a procedure in order to help you confirm that your Personally Identifiable Information remains correct and up to date or withdraw the processing of your Personally Identifiable Information should you wish to opt-out. You may access, update, delete your Personally Identifiable Information or communicate any concerns by sending an email to: privacy@carespanasia.com or communicate via any of our hotline numbers. Keep in mind, however, that there will be residual information that will remain within our databases, access logs and other records, which may or may not contain your Personally Identifiable Information. Your rights as a data subject will be enumerated in the Consent Form which you will be required to sign before you create your user profile. 


Changing our Privacy Policy 

Please note that we review our privacy practices from time to time, and that these practices are subject to change. Any change, update, or modification will be effective immediately upon posting on our Site and/or email transmittal. We will notify you of any material change to this Privacy Policy by posting a notice on our Site's homepage and the App and sending an email transmittal for a reasonable period of time following such update, and by changing the effective date.
